K-12 Professional Development Providers

State Approval of PD for Arkansas Educators

Thank you for your interest in providing professional development for K-12 licensed Arkansas educators! To obtain approval as a University of Arkansas Professional Development Provider and receive an Arkansas Department of Education Professional Development assurance number, please follow the steps below.

1. Email Suzanne Kucharczyk, Ed.D., to obtain the U of A Professional Development Provider Form.
2. Prior to completing the U of A Professional Development Provider Form:
   • • Please read ADE’s Rules Governing Professional Development and Learning Forward’s Standards for Professional Learning
     • Your U of A Professional Development Provider Form must explain how your proposed professional development event relates to both Arkansas Curriculum Standards and Standards for Professional Learning
3. No later than 30 days prior to the PD event, submit the completed U of A Professional Development Provider Form and all requested information to Dr. Kucharczyk
4. Receive notification of approval and a one-use ADE PD assurance number for the PD event
5. Create a spreadsheet of participants with participant information
6. Create and use a sign-in sheet, documenting the exact number of hours completed and ensuring participants sign in and sign out, both before and after a meal break
7. Create a certificate that includes the ADE PD assurance number and present it to each participant upon completion of the PD event
8. Within two weeks after the completion of the PD event, email the following items to Dr. Kucharczyk:
   • A digital copy of the spreadsheet with participant information
   • A scanned copy of the completed sign-in sheet
   • A digital copy of the certificate provided to all participants
